Your First Visit

If you are being referred by another provider, we will reach out to you and get you scheduled.
If you do not have a referral, you can email your contact information to admin@equitymentalhealth.com or fill out the Contact Form and we will reach out to you.

Once you have been contacted by your provider, you will be given an account in our patient portal.

If you have logged into the patient portal, you can access the intake forms there. If not, you can download them by clicking the purple download button above, printing them, and emailing them to admin@equitymentalhealth.com or faxing them to 480-718-8428.

Telehealth: You will receive an email with a link to join the virtual session with your provider. Your provider will spend the first session getting to know you, your concerns, and create a treatment plan that’s right for you.
In Person: When the provider arrives, they will spend the first session getting to know you, your concerns, and create a treatment plan that’s right for you.
